MidCPG e-forms
";MidCPG e-forms are used to submit the service information we need quickly and accurately.
E-forms can be accessed via this page or via URL links we have emailed.
Open the e-Form link, fill in each field, and attach any required files. Review your details, then select Submit.
You will receive a THANK-YOU alert once submitted successfully and will also receive a PDF report emailed to your dispensary email.
E-forms do have eligibility parameters set - ie: if you enter an ineligible patient you may not be able to submit the e-form.
It is important MidCPG has your correct dispensary email! If you encounter issues, please inform us info@midcpg.co.nz or call 07 858 5921.
